Consent & COVID-19
By engaging with the therapeutic services of Return Therapy, you understand that while the therapist is following all of the health and safety guidelines outlined by the CDC, the WHO, and the FSMTB, and that they are taking all reasonable precautions to clean and disinfect the treatment room and all the surfaces within the treatment room, there are no guarantees that you will not come into contact with covid-19. By signing the consent form, you acknowledge that you are aware of the risks involved and give consent to receive manual therapy from this therapist.
Unanticipated events happen,
especially now.
So, to be fair to all clients and out of consideration for your
therapist’s time, Return Therapy has adopted the following policies:
arriving late: appointment times have been arranged specifically for you. if you arrive late, your session may be shortened in order to accommodate others whose appointments follow yours. depending upon how late you arrive, i will determine if there is enough time remaining to start a treatment. regardless of the length of the treatment actually given, you will be responsible for payment in full, however i will do my best to satisfy the scheduled time.
cancellation: 24 hour advance notice is required when canceling an appointment. this way, someone else can have the opportunity to schedule. if you are unable to give me advance notice in a reasonable amount of time, you will be charged in full for your missed appointment.
no-show: anyone who either forgets or consciously chooses to forgo their appointment with no contact, will be considered a “no-show”. they will be charged the full amount for their missed appointment and future service will be denied until payment is made.

General Consent & Waiver
By purchasing the therapeutic services offered, you indicate that you agree with and understand the following:
it is your responsibility to consult a physician before participating in these or any massage services and you affirm that you have no medical conditions that would restrict you from participating in any of the services.
you agree to hold return therapy, and if applicable, its owners, therapists, representatives, and facilities harmless from any damage, whether tangible or intangible, that may happen to you while participating in the services offered.
you agree that return therapy offers its services with no guarantee of significant changes. you agree that any changes that occur, whether positive or negative, are the effects of your own personal choices.
you agree that participation in return therapy’s services is not a replacement for actual medical care, and that if you do experience medical issues, you will contact your doctor immediately.
you agree and verify that all of the information that you give to return therapy is accurate, up-to-date, and without the omission of any known medical issues.
you agree and verify that if you have omitted any necessary personal information, whether knowingly or unknowingly, you will hold return therapy harmless against all liability for any damages that may occur to you or to others because of your actions or inactions.
you agree to keep your therapist apprised of any changes or upcoming changes concerning your physical health and personal information.
you understand and agree that it is your responsibility to let your therapist know if you find yourself in any pain or discomfort before, after, or during the services.
you understand and agree that the services provided are not sexual in nature and any sexual innuendo, sexual or suggestive comments, or inappropriate touching will not be tolerated and will be cause for immediate termination of the session as well as your getting fired as a client. for privacy, blankets and draping will be used throughout all sessions.
if you do require medical treatment or attention while or after participating in return therapy’s services, you agree that the medical costs are yours and yours alone and hold return therapy blameless from any charges, fees, or costs that your conditions may incur.
the massage therapy consent & waiver will bind and be enforceable against you and all of your personal representatives. you agree that this massage therapy consent & waiver should be enforceable to the fullest extent of the law, and if any portion is held invalid, the remainder should continue in full legal force and effect.
you specifically acknowledge and agree that this document is not intended to be a general release, which would be limited under some state and local laws.
this massage therapy consent & waiver shall be construed and interpreted as broadly as possible in the applicable jurisdiction.
Further Acknowledgments
assumption of risk: you understand and are aware that your participation in return therapy’s services involves risks. these risks may lead to tangible or intangible harm, and you agree that they may result not only from your own actions but also from the actions of others. with the knowledge and understanding of these risks, you choose, of your own will and volition, to continue participating in the services provided by return therapy. you are also aware that there are risks that you may not have considered, yet you waive your right to any claims that may occur from these unconsidered risks and you choose, of your own will and volition, to participate in return therapy’s services.
covenant not to sue. you will not start any lawsuit or other court action against return therapy, or, if applicable, facility, nor will you join any such proceeding, including any claim for money damages. you acknowledge and agree that you are entering a covenant not to sue return therapy, llc in any capacity, including to hold return therapy liable for any injury, loss, or damage sustained by you or your property, even if it is due to return therapy’s negligence or omission. you also waive the right of any of your insurers' to make any such claim.
indemnification: you agree to defend and indemnify return therapy and any of its affiliates (if applicable) and hold them harmless against any and all legal claims and demands, including reasonable attorney's fees, which may arise from or relate to your use or misuse of return therapy’s services or your conduct or actions. you agree that return therapy shall be able to select its own legal counsel and may participate in its own defense, if desired.
representation: i am over 18 (eighteen) years of age, and am medically and physically able to participate in the massage services.
governing law: this massage therapy consent & waiver shall be governed by and construed in accordance with the internal laws of washington without giving effect to any choice or conflict of law provision or rule. each party irrevocably submits to the exclusive jurisdiction and venue of the federal and state courts located in the following county in any legal suit, action, or proceeding arising out of or based upon this massage therapy consent & waiver: king county.
you acknowledge that you have read the above massage therapy consent & waiver fully and you understand and agree to its contents. you understand and agree that this massage therapy consent & waiver forfeits any right, claim, or ability you have to hold the return therapy responsible for any tangible or intangible damages, loss of property, or loss of life that may occur during or after your use of the facilities and participation in return therapy’s services
